Operation Birthday is a story about Amanda, age six, who receives a nurse's kit as a birthday present. In a make believe sketch, she knocks her brother out, removes his intestines, and sews him back up again.

A young girl, wearing a blue and pink dress, is sitting in a garden. There is a birthday card on the table with a badge "I AM 6". The same girl is sitting in a front room in a chequered dress. Here she opens a present while sitting amongst other toys (presumably presents).
The present she is opening is a junior nurse's uniform and medical kit. She opens it and puts the uniform on. She sits with her medical kit. The instructions read: "First of all choose a willing patient".
Amanda points to her brother who is sitting on the floor playing with his toy garage. He quickly runs off. She points to her other brother who is sitting, reading a book called "The Playhour Story Book".
There is a bottle with the label "Chloroform poison" on a shelf. Amanda takes it down and proceeds to knock out her brother with some of the solution on a cloth. She arranges a small bed that was previously against a wall and puts her brother onto it and covers the lower half of his body with a blanket.
She goes to get a large knife out of a drawer in a cabinet. On top of the cabinet are a set of wooden owl salt and pepper shakers which have been used in Smith's other films. She kneels next to her brother on the bed and starts to carve his stomach with the knife. She pulls out lengths of sausages i.e. intestines.
She then sews up his stomach and puts a bandage round it. She takes a bottle of "knock in drops" and syringe, injects his arm, he wakes with a start. He quickly gets off the bed, rolls around then leaves the room. Amanda laughs. Sausages/Intestines spell out "The End".
